Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment, as well as co-payments for HMOs and deductibles for PPOs.

We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, and Visa, MasterCard, American Express and Discover credit cards.

We also extend a 30% discount to patients who self-pay in full at the time of treatment.

We are pleased to offer no-interest, in-house payment plans with low monthly payments to make your treatment more affordable. This flexible payment option allows you to pay for your treatment over a period of time so you can focus on what matters most – staying healthy!

Insurance Policy

Our office staff will be glad to assist you with your insurance coverage. When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment.

We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 45 days. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.

Uninsured Self-Pay

Springfield Nephrology Associates is dedicated to assisting uninsured patients with receiving the highest quality healthcare at a manageable cost. If you currently do not have insurance we offer a discount of 30% if paid at the time of service. If you are unable to pay at the time of service you will be directed to our patient advocate to discuss payment arrangements or possible financial assistance with your bill.

Financial Assistance for Uninsured or Underinsured Patients

To ensure that all patients have an equal opportunity to address his or her outstanding balance within the confines of their household income, Springfield Nephrology Associates, Inc. offers financial assistance to qualifying patients, whether seen in the hospital, dialysis unit, or clinic. If you are unable to pay your bill at time of service, or are unable to pay your balance due after insurance, you may qualify for financial assistance. Our financial assistance program is based on a sliding scale in accordance with Federal Poverty Guidelines. Qualified applicants will receive a discount ranging from 10% up to 100% of the outstanding self-pay balance and any future unpaid balances for the period of 12 months from the date of approval. All applicants will receive notification of approval or denial within 30 days after completing the application. To ensure that your application is expedited in a timely manner, please read the instructions and be prepared to submit the appropriate documentation with your application.
